Dear Faculty and Staff,



SUNY Plattsburgh is committed to the health and safety of our campus
community. All employees who actively report for on-campus work are
expected to participate in the pool testing program.  Please reference the
schedule below, based on the first letter of your last name for your
intended time of testing.  If you are unable to be tested at your
designated time, you are welcome to come at another time within the
available options (between 9am – noon and 1 – 3:30pm on Tuesday, January
19th).



WHO MUST PARTICIPATE:

-If you are reporting to campus to complete some or all of your work
obligation and are an employee represented by CSEA, UUP, PBA and PEF you
are subject to surveillance testing under specific Memorandums of
Understanding (MOU) recently reached between SUNY and each bargaining unit.
MC employees are also deemed subject to testing. As such, attendance is
considered mandatory even if the days you report to campus are not normally
on Tuesday.



WHAT TO BRING:

-Plattsburgh ID card

-Banner ID number

-COVID test account password

-Your personal mobile device (e.g. smartphone or tablet) if you have one



WHO IS EXEMPT:

-Employees who are fully remote and do not report to work in person on
campus.

-Employees at the branch campus (SUNY Plattsburgh at Queensbury).

-Employees who work the night shift within Maintenance & Operations or
University Police.

-Graduate Assistants who are included in the student pool testing.

-Anyone who has tested positive for COVID-19 within the last 3 months



ALTERNATIVE OPTIONS:

-While every effort should be made to attend the employee pool testing on
Tuesday, January 19, those unable to attend can be tested off-campus at
their own expense, by their health care provider, pharmacy, or other
COVID-19 testing facility.  In order to meet this requirement, the test
must be conducted within 24 hours of our employee pool testing (January 18
– January 20) and the results must be sent to Human Resource Services.



WHAT TO EXPECT:

Surveillance pool testing is easy and involves you swabbing your mouth.  To

Participating in the pool testing is simple; please do the following:



Step 1: Before your testing day, you must create a COVID-19 surveillance
account and register online by using this link -
https://www.suny-covid-test.com/. You will be asked to create a password
for your account, enter your banner ID number and health insurance
information. If a pool result is positive, Upstate Medical University will
immediately re-test the individual samples within the pool to identify
which person has COVID 19. Upstate Medical University will seek
reimbursement from your health insurer should they need to test your
individual sample.



Step 2: Employee testing is scheduled for Tuesday, January 19, 2021.  You
will need to come to Algonquin Dining Hall (next to Adirondack hall)
according to your last name as follows:



● Last name begins with A - CH scheduled for 9:00 – 10:00am

● Last name begins with CI - F scheduled for 10:00 – 11:00am

● Last name begins with G – K scheduled for 11:00 – 12:00 noon

● Last name begins with L – M scheduled for 1:00 – 2:00pm

● Last name begins with N – R scheduled for 2:00 – 3:00pm

● Last name begins with S – Z scheduled for 3:00 – 3:30pm



Remember to complete the registration process online before coming to
Algonquin Dining Hall for your test.



Upon arrival, follow the signs to the pool testing entrance. Please
maintain six (6) feet of distance from others and wear your mask. The
testing process is quick and easy, and involves swabbing your mouth, not
your nose.



Step 3: You must bring your Plattsburgh ID card, Banner ID number, COVID
test account password and your personal mobile device (e.g. smartphone or
tablet) to the testing site.  Employees without access to a mobile device
will be assisted with the final stage of the registration process, at the
testing site. However, it is required that all employees complete the
initial registration process online (Step 1) in advance of arrival at
Algonquin Hall.



On the Day of your Test:

● No eating or drinking prior to the test: You must not eat or drink
anything, including chewing gum, mints or lozenges for 30 minutes prior to
your test;

● No Teeth Brushing and Mouthwash: You should not have brushed your teeth
or used mouthwash within the past 3 hours of reporting for your test;

● No Smoking/Vaping: You must abstain from smoking, vaping or using
smokeless tobacco products for 30 minutes prior to the test.



We appreciate your ongoing efforts to keep our campus community safe and
healthy. Please remain vigilant, abide by the Cardinal Pledge, comply with
the Social Distancing and Face Covering Policy and avoid nonessential
travel.
