Hospital Program Administrator Name Change, Effective January 1, 2024

Effective January 1, 2024, Empire BlueCross, the administrator for the
Empire Plan Hospital Program, will change its name to Anthem Blue Cross.

There will be no changes to coverage or the level of support due to the
