2025 Annual Option Transfer Period Information

The Option Transfer Period is here!

The 2025 NYSHIP Option Transfer Period runs from Friday, November 29, 2024
through Tuesday, December 31, 2024

During the Option Transfer Period

you may make the following changes to your New York State Health Insurance
Program (NYSHIP) option for 2025:

   -

   Change to the Empire Plan
   -

   Change to a NYSHIP Health Maintenance Organization (HMO)
   -

   Change to the Opt-Out Program (if eligible).  Please note that it is no
   longer necessary to reenroll in the Opt-Out Program each year.
   -

      If an employee has coverage under another employer-sponsored health
      insurance program, they may be eligible to opt out of their
current NYSHIP
      coverage in exchange for an incentive payment during the Option Transfer
      Period. No action is required for current Opt-out enrollees who are still
      eligible and wish to remain in the Program for the 2025 plan year. Note:
      Employees who are represented by UUP are not eligible to participate in
      this program.
      -

      Eligibility Requirements: To be eligible for the Opt-out Program in
      2025, you must have been enrolled in any NYSHIP plan by April 1, 2024 (or
      the first date of NYSHIP eligibility if that date is later than April 1),
      through the end of the plan year. Other coverage must be through
employers
      other than the State of New York.
      -

      If electing the Opt-Out Program a completed PS409, found here,
      <https://www.cs.ny.gov/forms/ps409.pdf> and proof of the other
      employer-sponsored coverage are also required.


Other changes permitted during the Option Transfer Period include:

   -

   Change from family to individual coverage (whether you have a qualifying
   event or not);
   -

   Voluntarily cancel your coverage (whether you have a qualifying event or
   not);
   -

   Change your Pre-Tax Contribution Program (PTCP) election; changing from
   a pre-tax to a post-tax deduction or vice versa.

Note: Elections to participate in the Pre-Tax Contribution Program (PTCP)
cannot be made while enrolled in the Opt-out Program as there is no health
insurance premium deduction.



No action is required if you wish to keep your current health insurance
carrier.

You will need to complete a PS404, found here
<https://www.cs.ny.gov/forms/ps404.pdf>, and return to Human Resource
Services (via fax at 518-564-5060 or email at hrs at plattsburgh.edu) prior to
the deadline of December 31st.


Employees who request to enroll in NYSHIP coverage or add previously
eligible dependents to existing coverage are still subject to NYSHIP’s late
enrollment rules. This means that employees who request enrollment without
a qualifying life event and after their 28-day waiting period will be
considered late enrollees.

New York State's Department of Civil Service has announced the 2025
biweekly health insurance rates as follows:

Biweekly rates for employees who are Management/Confidential or represented
by CSEA, PEF, or UUP in salary grade 9 or below (or annualized salaries
less than $50,885 for UUP or Management/Confidential employees)

equal to $50,885 or more for UUP or Management/Confidential employees)

dependent upon the county that you live in.  If you need help deciding
