Health Screening Form Information Collection

 The following is being sent on behalf of R. Lizzie Wahab, VP for
Enrollment and Student Success

Students/Colleagues,

In support of the SUNY mandate of weekly COVID-19 testing and to assist in
our efforts to quickly notify positive persons, we will be conducting a
one-time verification/update of employee and student contact information
through the completion of new fields in the Daily Health Screening Form.
This collection is for information that will be only used by the health and
safety team for COVID purposes.  The collection will go "live" on Thursday,
February 4th.
