[Important Information for Students] Student COVID-19 Requirements and Testing - Fall 2021 Important Information

Dear Students,

To help you prepare for our health and safety requirements for this fall we
are sharing the following information regarding mandatory COVID-19 testing.


   -

   Week 1 (August 30 - September 2) ALL STUDENTS who are coming to campus
   for any reason (in-person classes, living in the residence halls, student
   organization meetings, etc.) MUST PARTICIPATE IN COVID-19 CAMPUS POOL
   TESTING! The Algonquin Hall test site will be open Monday, August 30,
   Tuesday, Aug. 31 and Thursday, September 2 from 9 a.m. – noon; 1 – 4 p.m.
   -

   A one-time registration is required before your first test. Register at SUNY
   COVID-19 Testing <https://www.suny-covid-test.com/>. You will be asked
   to create an account. It will require you to create a password, enter your
   banner ID number and health insurance information. Remember your password
   or use a secure password manager, as you will need it to log in to your
   account prior to testing.

      You will need to come to Algonquin Dining Hall (next to Adirondack
      hall) according to your last name as follows:
      -

         Last name begins with A - C scheduled for 9:00 – 10:00am
         -

         Last name begins with D - F scheduled for 10:00 – 11:00am
         -

         Last name begins with G – K scheduled for 11:00 – noon
         -

         Last name begins with L – M scheduled for 1:00 – 2:00pm
         -

         Last name begins with N – R scheduled for 2:00 – 3:00pm
         -

         Last name begins with S – Z scheduled for 3:00 – 3:30pm
         -

   Starting Sept. 6 - All unvaccinated or partially vaccinated students who
   are coming to campus for any reason (in-person classes, living in the
   residence halls, student organization meetings, etc.) must participate in
   testing. ONLY students who provide proof of vaccination through the SUNY
   Portal SUNY Portal <https://suny.edu/StudentVaccinationPass> will be
   exempt from this requirement.

   Additional Mandatory Testing of ALLStudents. Mandatory weekly COVID-19
   testing is required of all unvaccinated (or partially vaccinated) students,
   including those with medical or religious exemption.  However, mandatory
   periodic COVID-19 testing is required of ALL STUDENTS.  Dates for
   mandatory testing are:
   -

      Week of October 11th and the week of November 29th
      -

   Location and Hours of Operation.  Algonquin Hall. All students must
   participate in COVID-19 testing.  Starting Sept. 6, the Algonquin Hall test
   site will be open Mondays and Tuesdays from 9 a.m. – noon; 1 – 4 p.m. The

   Testing Opt Out. Vaccinated students may opt out of testing by providing
   proof of vaccination through the SUNY Portal
   <https://suny.edu/StudentVaccinationPass>

   Home Test Kits. New this fall, students will be given their first home
   test kit after attending testing on Monday, August 30, Tuesday, Aug. 31 or
   Thursday, September 2 at Algonquin Hall.  Thereafter, students will be
   given a new test kit after dropping off their completed test kits each
   week.
