[Important Information for Employees] Revised COVID-19 Guidance - Affirmation Required

***Breaking News!!***

The NYS Department of Health has issued Revised Guidance for State Agencies
during the COVID-19 Public Health Emergency.  Given New York’s significant
progress in COVID-19 vaccinations and decline in new COVID-19 cases, we can
begin to return to normal operations.

A brief summary of the new provisions include:

-Fully vaccinated individuals do not need to be socially distanced and
unvaccinated/unknown vaccination status individuals need to maintain six
feet of social distance.

-Fully vaccinated individuals do not need to wear masks and
unvaccinated/unknown vaccination status individuals need to wear masks.

All State employees must affirm that they have read the guidance in its
entirety, understand its provisions, and will adhere to its requirements.  The

Print, sign, and return to Human Resource Services via scan/email, regular
mail, fax, or hand delivered to 912 Kehoe.

Our third party affiliates, CAS, Chartwells, Research Foundation, and
others, do not have to complete the attestation.

We are in the process of updating the Social Distancing and Face Covering
to reflect these changes, as well.

While the plans for the fall semester are still being developed, those
employees currently reporting to campus must complete the attestation form
as soon as possible.  Employees working remotely through July 2nd will be
required to submit the attestation form no later than July 9th.
