The Governor’s Office has announced new guidelines for Fully Vaccinated
people and SUNY has issued 2021 Summer and Fall Semester Planning updated
COVID-19 protocols. While the impact of several of these measures are still
being finalized, we are pleased to announce the following change to our
weekly mandatory COVID-19 surveillance pool testing.

Effective immediately, fully vaccinated employees (including auxiliary
workers and vendors with a regular on-campus presence), who wish to
disclose their COVID-19 vaccination status, will no longer have to
participate in the campus’ weekly COVID-19 surveillance pool testing.

In order to be exempt from testing, vaccinated employees will need to
provide proof of vaccination via the Vaccination Attestation Form
<https://docs.google.com/forms/d/e/1FAIpQLScqs_CGtt3U--Cyc1N99GYJKJ8Ac5CA7NVPsERY9HjjZKhZkg/viewform>
. A Acceptable proof is a copy of your completed CDC vaccination card or a
screenshot of your NYS Excelsior Pass <https://epass.ny.gov/home>. This
information will be kept confidential and will not become part of your campus
employment records.

Those employees that do not wish to disclose their vaccination status or
those employees that are not Fully Vaccinated must continue to participate
in weekly COVID-19 surveillance pool testing.

“Fully Vaccinated” is defined as:

   - two weeks after the second dose in a two-dose vaccination series, such
as the Pfizer-BioNTech or Moderna vaccines, or,
   - two weeks after a single dose vaccine, such as Johnson & Johnson’s
   Janssen vaccine.

Please note that all employees will be expected to continue to
complete the COVID-19
daily screening form and, if reporting physically to campus, conform
to our Social
Distancing and Face Covering Policy
