</div><div>Dear Faculty and Staff,<br>
<br>
SUNY Plattsburgh is committed to the health and safety of our campus community. All <span>employees</span> who actively report for on-campus work are expected to participate in the pool <span>testing</span> program. Please reference the schedule below, based on the first letter of your last name for your intended time of <span>testing</span>. If you are unable to be <span>tested</span> at your designated time, you are welcome to come at another time within the available options (between 9:00am – 12:00 noon and 1:00pm – 3:30pm on Wednesday, November 18th).<br>
<br>
WHO MUST PARTICIPATE:<br>
-<span>Employees</span> represented by CSEA, UUP, PBA and PEF who report to work in person on campus to complete some or all of their work obligation are subject to surveillance <span>testing</span> under specific Memorandums of Understanding (MOU) recently reached between SUNY and each bargaining unit. MC <span>employees</span> are also deemed subject to <span>testing</span>. As such, attendance is considered mandatory even if the days you report to campus are not normally on Wednesday.<br>
<br>
WHAT TO BRING:<br>
-Plattsburgh ID card<br>
-Banner ID number<br>
-COVID <span>test</span> account password<br>
-Your personal mobile device (e.g. smartphone or tablet) if you have one<br>
<br>
WHO ARE EXEMPT:<br>
-<span>Employees</span> who are fully remote and do not report to work in person on campus.<br>
-<span>Employees</span> at the branch campus (SUNY Plattsburgh at Queensbury).<br>
-<span>Employees</span> who work the night shift within Maintenance & Operations or University Police.<br>
-Graduate Assistants who are included in the student pool <span>testing</span>.<br>
-Anyone who has <span>tested</span> positive for COVID-19 within the last 3 months<br>
<br>
ALTERNATIVE OPTIONS:<br>
-While every effort should be made to attend the <span>employee</span> pool <span>testing</span> on Wednesday, November <span>18</span>, those unable to attend can be <span>tested</span> off-campus at their own expense, by their health care provider, pharmacy, or other COVID-19 <span>testing</span> facility. In order to meet this requirement, the <span>test</span> must be conducted within 24 hours of our <span>employee</span> pool <span>testing</span> (November 17 – November 19) and the results must be sent to Human Resource Services.<br>
<br>
WHAT TO EXPECT:<br>
Surveillance pool <span>testing</span> is easy and involves you swabbing your mouth. To understand what to expect during the pool <span>testing</span> process, please watch this brief video.<br>
<br>
Participating in the pool <span>testing</span> is simple; please do the following:<br>
<br>
Step 1: Before your <span>testing</span> day, you must create a COVID-19 surveillance account and register<br>
online by using this link. You will be asked to create a password for your account, enter your banner ID number and health insurance information. If a pool result is positive, Upstate Medical University will immediately re-<span>test</span> the individual samples within the pool to identify which person has COVID 19. Upstate Medical University will seek reimbursement from your health insurer should they need to <span>test</span> your individual sample. <br>
<br>
Step 2: <span>Employee</span> <span>testing</span> is scheduled for Wednesday, November <span>18</span>, 2020. You will need to come to Algonquin Dining Hall (next to Adirondack hall) according to your last name as follows:<br>
<br>
● Last name begins with A - CH scheduled for 9:00 – 10:00am<br>
● Last name begins with CI - F scheduled for 10:00 – 11:00am<br>
● Last name begins with G – K scheduled for 11:00 – 12:00 noon<br>
● Last name begins with L – M scheduled for 1:00 – 2:00pm<br>
● Last name begins with N – R scheduled for 2:00 – 3:00pm<br>
● Last name begins with S – Z scheduled for 3:00 – 3:30pm<br>
<br>
Remember to complete the registration process online before coming to Algonquin Dining Hall for your <span>test</span>.<br>
<br>
Upon arrival, follow the signs to the pool <span>testing</span> entrance. Please maintain six (6) feet of distance from others and wear your mask. The <span>testing</span> process is quick and easy, and involves swabbing your mouth, not your nose.<br>
<br>
Step 3: You must bring your Plattsburgh ID card, Banner ID number, COVID <span>test</span> account password and your personal mobile device (e.g. smartphone or tablet) to the <span>testing</span> site. <span>Employees</span> without access to a mobile device will be assisted with the final stage of the registration process, at the <span>testing</span> site. However, it is required that all <span>employees</span> complete the initial registration process online (Step 1) in advance of arrival at Algonquin Hall. <br>
<br>
On the Day of your <span>Test</span>:<br>
● No eating or drinking prior to the <span>test</span>: You must not eat or drink anything, including chewing gum, mints or lozenges for 30 minutes prior to your <span>test</span>;<br>
● No Teeth Brushing and Mouthwash: You should not have brushed your teeth or used mouthwash within the past 3 hours of reporting for your <span>test</span>;<br>
● No Smoking/Vaping: You must abstain from smoking, vaping or using smokeless tobacco products for 30 minutes prior to the <span>test</span>.<br>
<br>
